You have to apply for a spouse settlement visa. Your husband has to sponsor you and HE has to fulfill financial requirements.

Depending on how he intends to sponsor you he will need to provide payslips, bank statements etc to show he earns enough to support you.

You don't need to provide the numbers of all your old passports.

Depending on what name you are applying under, you provide that passport number as your current one.

In addition to financial documentation he will need to provide proof of accommodation. Other docs required are your marriage certificate, passport itself, Appendix 2, and printed copy of VAF4a form. (not a total list of docs you might want to send to prove your relationship)
