It's rare that the French government puts out English translations of any of their forms.

The one you cite is: DEMANDE D'AUTORISATION DE TRAVAIL
POUR UN SALARIÉ ÉTRANGER - CONTRAT DE TRAVAIL SIMPLIFIÉ

Basically, it looks like you need to have some pre-existing authorization to live in France (i.e. a carte de séjour). If you're a UK citizen, I don't think this should be necessary, as you have an automatic right to work in France, based on your UK nationality.

Have done some hunting around online and all I have been able to find out is that this form is apparently used for authorizing domestic helpers to enter and work in France. Are you going to be submitting the form to the DDTEFP here in France? Or does your husband need it for the local French consulate wherever he is now in order to apply for the carer's visa?

As far as I can tell, I can't find any English version of this form, nor any instructions or information specifically about the form in English - other than some references to it with regard to getting authorization for a domestic worker to accompany the family with which he or she has been working abroad. You could try asking at the French consulate (assuming you are currently located outside France) or if there is an Alliance Française anywhere in the area, someone there might be able to help you with the form.
