Hello

if a couple are both working and making money, do they apply for their taxes jointly as a family or separately? In other words, if he makes X and she makes Y, do they add up X and Y, and get taxed as per the (X+Y) bracket? or each of them is taxed separately?

Separately. Separate returns, no income sharing. Might not be fair but it is as it is. Note though that social security payments are calculated taking into account the circumstances of both partners. Thus if you are unemployed and partner is still working you generally won't be able to get any benefits.

Depends. If one is on a very high income and the other very low, less tax would be paid if the income was to be averaged. But not going to happen here as the government is trying desperately to increase revenue.
