I am preparing statutory Declaration for skill assessment by ACS. I have following questions-

1. Is it mandatory to include my employee # of companies I have worked with in the statutory declaration?

2. Do I need to provide email id and Phone no. of the colleague in the document who is writing my affidavit?

2. Yes, it is better to include business card or contact details of the colleague so that ACS can contact your colleague in case they want to verify SD content.
