I went with 100 rs stamp paper from senior collegue and it went fine.
You can also write your own wording by following any company ref letter.
I have no access to attach here it seems.

For the statements you can go through the attached document and choose the words from here. Do not copy like to like statements, use the doc as a template.
Ensure you give accurate details of the supervisor/senior colleague (name, ph no, email) and signature of the person is mandatory

It has to be issued my your manager or team leader and as a last resort colleague of the company where you worked
Not some random guy who will vouch for you
