I am an American in the UK on a Tier 2 visa.

My English partner and I plan on getting married soon, but happen to have the same last name. It's a Polish name which is quite unusual in the UK, but not-so-unusual in Poland.

We are not related and can trace this up to four generations back.

Should we include our respective birth certificates to prove that we are not siblings, or wait to see if they request this information?

Thank you in advance.

Yes, that's a good idea. Though a lot of Miss Smith marry Mr Smith in UK!
