Pg. 9 ... 1.7 "Any other names by which you are or have been known."
My passport ( good for 7 more years ) is in my old name as is my initial spousal visa but most of the mail I will be submitting is now in my new married name. Also bank accounts are in my new married name.
Do I put my married name in for question 1.7 or leave it blank?
Pg. 20 ... 6.5 ... How do you keep in touch with your sponsor? Do I put in N/A or we live together, since question 6.6 asks do we live together ?
Pg. 21 ... 6.8 ... We moved to a new house so will be listing two addresses for this question. Where it says date from ... Then to ... Do I leave the "To" date blank or do I put in the date of our appointment? I suspect we will be in our new house until we die.
Pg. 24 ... 6.23 ... They ask type of ceremony.
Our wedding was done in a house with a justice of the piece performing the ceremony? What type is that?
