The UK visas and immigration website isn't really clear with this - I just want to know, how do we go from fiancee visa, getting married, to spouse visa. Would my future wife need to go back to the states to apply for a spouse visa, since the fiancee visa says that it shouldnt be used with the intent to stay in the UK? I'm somewhat confused... Would it be easier for us to marry in the USA?

Perhaps you are looking at a marriage visit visa which is for those who wish to marry in the UK but don't intend to live in the UK.

If you want to marry in the UK and remain in the UK you would apply to join family as a fiance. We generally call it a fiance visa. It gives you 6 months to marry an after you marry you apply for FLR (M) which is further leave to remain. You apply from within the UK.

If you marry in the US you will save yourself the cost of 1 visa and you will apply to join family as a spouse.
