1. Do you have TC given from college? It has clearly mentioned date of joining and leaving. If you don't have it should be OK to give some rough dates. shouldn't be a big issue
2. I think there is a separate section in Form 80 asking where you lived in past 10 years. In 176 you might give current residential address and contact details.
3. No, if they are under other family member they are waived off from these 2 things. If they are under non-migrating dependents yes they have to undergo pcc/meds and the results of their tests will have impact on your 176 visa.

1. after applying online check the 'document checklist' section. if it asks for it fill and upload else let wait for CO request on this.
2. Simply upload your docs and wait for CO allocation.
3. yes immediately after applying there will be few links to download the pdf of application form. do it else later you may not be able to retrive the application and will be very difficult to know what information you filled. better to take a copies and store in diff location as one of the member pc crashed and lost all his info. simple but very useful precaution

5. in fact all doc to be uploaded in pdf which they prefer.
