Default Flrm Application Confusion and Adding Previous Known Names

I am currently in the UK as the applicant of Souse Settlement Visa. I am in the middle of my online FLRM extension application.


I am hoping that this photo will clarify my situation. Most married women have a Maiden name, and a married name. My wife has a Maiden name, a previous married name and a new married name. She has been known by all 3 names. Her passport is in her previous married name, not her current name.

The attached photo shows the scenario of adding previous names. Bottom line is, Do we add my wife's other known sir (last) names as in the photo or is this simply a place to add previous passports she may have had? I am asking anyone that has seen this situation during their application process to please help me clear this up. Again I give UKVI credit for making portions of their application process as complicated as possible without making it impossible. Thank you in advance
