Default Problem with name change affidavit

My Birth certificate has a different name than the passport and all other documents. So I took a name change affidavit from a JP.
I have two problems with that.

1. In the affidavit he has mentioned my permanent address which is in my ID card, which is my parent's address. I do not have my own permanent address as I am living in a rented place with my husband.
Could this cause an issue in the visa stage? Should I change the address to my current residence address?

2. This JP is my translator as well. When look at the seal at the affidavit, instantly what notices is that he is a translator. His JP position is mentioned in very small letters at the bottom in the seal. Will this cause an issue in the visa stage(my concern is whether case officer will not notice that he is a JP and identify my affidavit as invalid)

Your help and suggestions are very much appreciated.
