What type of Folder did everyone send the copy of the application and the bio receipt and supporting documents in?

None - just two separate bundles - one clearly the copies and the other the originals, all sections clipped separately and flagged.

I used binder clips for each section (my info, his info, financial info, etc), and then big elastic bands to contain the whole bundle. Then the bundle was put into a USPS padded priority envelope and handed with a huge sigh of relief to the Post Office clerk (along with $15USD).

Some people go all out with sleeve protectors (don't hole punch anything, not originals or copies), tabbed binders, and several other ways, it's been really interesting reading how some of the packages have gone to the UKBA NYC. Depending on the size of the bundle, there have been applicants who've needed the USPS box to ship their application and supporting documents.

ETA-important: in the US where you'll be submitting your hard copies, the photocopy needs to be directly behind each original.

Another Question....
One of the questions on the application under the section about Passport Information ask "Place of Issue". My new passport with my new married name, which I just received last week, doesnt say a place of issue.
????
Just wondered what I should put there.

Does the document indicate the city in which you applied or did you have to mail the application off somewhere? If so, put that place down.

My second passport once said "Hull" because I had to mail the application off to the city of Hull, in the province of Québec. The current one says "Surrey," because I went to a passport office near where I live in a city called Surrey. When I lived in the Caribbean and had to get my passport replaced (once because it was about to expire and again a few months later because I thought that I'd thrown it out by accident after a trip to Cuba.... very expensive mistake that, especially since the passport turned up 5 minutes before I went to collect the new one).... when I went to get it replaced, the application had to go to the Consulate in Kingston, Jamaica, as I was w*rking outside of Canada at the time and there was only an honourary Consulate in the Cayman Islands where I was living.

I had to mail it into the National Passport Processing Center in Philadelphia, PA. So should I put Philadelphia, PA, USA down for Place of Issue?

On a US passport (important distinction

Whatever is listed under the 'Authority/etc' header (see below) is the authority issuing your passport.

Look on your biopage, on the right lower corner for the words in bold 'United States Department of State'. It's under a lighter toned header 'Authority/Autorite/Autoridad'.

In the old days, the place listed under 'Authority/etc' would have been Philadelphia, Los Angeles, NOLA, Charleston, or another passport issuing hub in the US.

Now it's the State Department, and that's what you put on that line.

Yea, neither my old one which was issued in 2007 or my new one which was issued last week has a Town on it. My sister's which was issued a couple of years before my first one in 2007 does have a town, New Orleans...

So what do I put for Place of Issue?

US Department of State. I know, I thought it should be a place as well.

When I worked for the US government (I'm a retired bureaucrat, lol, worked in stats) and they changed the way they organise US passports (around 2002) the question came up whilst doing recruiting for 2010 Census supervisors. The correct answer is 'USDS'-only write it on the form as US Department of State.

OK, right, I thought, but surely for the UK visa application the correct answer is something else...so I clicked on the little info pop-up option on the form, and hey presto, USDS is what you put.

I got my visa
