Lightbulb filling the skill assesment form

I have 3 queries to be asked:

1)I m going for filling the ACS thru mail(getting the DD) so wat shall i fill in the section made for payments on page 1(attached image1-Payment)...shall i leave it blank..

2)What shall i fill for the education section(attached image2-Education)...10 ,12 & graduation..all three of them??or only 12 & Graduation??

3)I have worked in the same company for 4 years but with different profile like
1)JAVA/J2EE (past)
2)Weblogic Integration(past)
3)Process/business analysis(current)
so while I m filling the skill assesment form(see attached image3-Experience)I shall fill in one row stating my only company(i have only worked for 1 company for 4 years)...and in that fill all the profiles that worked in 4 years??

pankaj, u got to relax my friend.. we post whenever we can, u cant get a reply immediately after posting it.

anyway,

1. it says clearly, if paying be credit card.. no mention of DD..so it has to be blank

2. it has to be 10th, 12th, graduation..

3. both ways are right, u can make it position wise since i guess u hv worked in just one company. the company name can be the same, otherwise u can put the details of each position on the side (last column)

i am not sure about the credit card, but yes they do have the section for the card, i am sure u can make it using ur card. but i suggest a draft when u r sending it by post/courier

the norm is registered post since u get a confirmation that the parcel has been recieved, and u can track the same. but i know a lot of people who did use courier services.

we have an agent, who was in india, very much in gurgaon, so we met him and gave the parcel to him by hand. but for our visa we couriered the papers using DHL and it was freaking xpensive..

and why i say relax, it is not possible for everyone to be online when u post a query. With me it is different, i work form home and i am online most of the day but for those in AU or other locations, there is a time diff., so u cant really get a reply there and then.

Business Analyst on the form, but when you fill in the detail of the position you will say Process Designer as the TITLE, then fill in details that support the definition of a Business Analyst.

Remember, make it easy for the ACS person to go 'tick' for Business Analyst and that is what they will do.
