Few queries re 1276 form fill-up

Unhappy Few queries re 1276 form fill-up

Hi all,
I have got the ACS assessment result last August and sat for IELTS expecting result in next week. Thought starting to fill up the forms and arranging the necessary papers would be great to do right now.
However, feeling some difficulties and need ur expert opinion on those.

1. Do I need to send the original copy of ACS and TRF, or can I send the attested/notarized copy? In that case I can retain the original ones if needed further.

2. section (13) [details of countries of residence] - as i have never left the country for long (12 months) - can i just put in my current month - or leave it blank?

3. Q - 25 - Nominated Occupation: i got result as '2231-79' from ACS with title "Senior Officer" according to my current job title. However - it is not in SOL, rather i applied for 'Computing Professional - specializing in C#' with relavant docs. Can I put that?

4. The passport of my wife got renewed, with the old passport attached in the back of new one by staples. Now in section (42 - if holds more than one passport) - should I put the details of other passport? Note - it can be vital as it holds some VISA stamps traveling with me in other countries. (evidence - relationship)

5. Q - 66, Part H - including other family members
I have not put in any other members of my family in else where as they are not my dependent nor migrating with me. Do i have to put them in this section?

6. Evidence of relationship:
I am married for 4.5 years with a recent baby. The docs i am sending: a. marriage certificate b. bank paper (wife as nominee) c. wedding pics d. honeymoon pics e. casual family pics f. (we have VISA stamps in passport for traveling together 4/5 countries) g. her PP and Voter ID has my name as spouse

are they sufficient for sending now with the application?

sorry for long list of queries. i really need some kind of assistance as I am doing it all myself.

1. you need to send certified copies only ... DO NOT send originals
2. I also didn't stay in any other countries for more than 12 months, however I lived in 4-5 different cities & rented places in my own country. So i mentioned all of them date wise
3. You should put whatever you got from ACS ... However, you can attach extra sheet along with form and mention this particular thing with Q-25
4. Yes you must put all passport details, if not suffcient space, attach extra sheets and also provide them certified copies of all relevant pages of passport, no matter whether it is old or new.
5. Yes, you nevertheless have to mention details of your family members, including your parents,parents-inlaws, your brothers and sisters and your wife's brothers and sisters. It is mandatory.
6.submit whatever you can .. the list what you have prepared is sufficient to show evidence of your relationship.
